Documentation Requirement and Submission Process
Following is the list of documents for Ground Floor structures.

Proof of on or before date 1.1.2000 along with name of hutment dweller mentioning hutment number or mention of definite location (e.g. chawl, road etc. residential address); - -1) Certified abstract of final electoral roll published in year 2000 or before by office of district collector. Or -2) Documents/records/bills of electric connection given by authorized electric company to that hutment Or -3) Enumeration form given by state government under "Identity Card Scheme, 2001 for eligible hutment dwellers" implemented with reference to directive dated 11 July, 2001 Or 4) Proof of assessment of property tax of hutment by municipal corporation/city council. -5) Non-agricultural permission from revenue department of state government or permission regularizing non-agricultural use and receipt of non-agriculture tax/non-agriculture penalty paid at that time Or -6) Share certificate certified by Assistant Registrar, Cooperation of slum cooperative housing society registered on or before 1.1.2000. Or -7) (a) Permit for agency (gumasta)/mess or (b) Restaurant license/tax receipt in that context obtained from municipal corporation/municipality/city council on or before 1.1.2000 mentioning hutment number or showing its definite location in respect of hutment being used for professional or industrial nature use or being used besides for residential purposes. |

The proof of 1.1.2021 or prior to the same having the mention of hut number including the name of the hutment dweller or having mention of the exact place of the hut (for example, Chawl, Road, etc. residential address). .. 1) Certified extract of the Final Voters List of the year 2011 or of the period prior to the same, published through the Office of the District Collector. Or 2) Documents/Records/ Bills of electric connection given by Authorised Electric company in the said hut. Or 3) Proof of Property tax assessment of the hut made by the Municipal Corporation / Nagar Parishad. Or 4) The Non-Agricultural permission or the permission of regularising non-agricultural use, of the Revenue Department of the State Government and the receipt of the amount of non-agricultural tax / Non-agricultural fine paid at that time. Or 5) Share Certificate of the Slum Cooperative Housing Society registered on 1.1.2011 or prior to the same certified by the Assistant Registrar, Cooperative Societies. Or 6) In case of huts in use of Commercial or Industrial nature or in residential use including the same, (a) Shop and Establishment Licence/ Khanajaval or (b) Restaurant Licence/ the receipt of tax paid in that connection of the Municipal Corporation/ Nagarpalika/ Nagar Parishad on 1.1.2011 or prior to the same, having mention of the said hut number or showing the exact place. |
For Rental Housing..
- Category 1: Slumdwellers residing on the ground floor of certified slums after January 1, 2011.
- Category 2: Slumdwellers residing on the upper floors of certified slums
The head of the family living on the upper floor of the slum before the cut-off date must submit the following documents in priority order for the verification of the slum and identification of the family head.
- Priority 1: An electricity bill in the name of the family head, mentioning the upper floor of the slum, dated before the cut-off date and a recent bill.
- Priority 2: A registered rent agreement mentioning the upper floor of the slum. At least one agreement must be from before the cut-off date, and another recent (latest) agreement. The tenant, according to the most recent registered rent agreement, will be considered the "identified family head." - Priority 3: A registered purchase agreement (for all owners) and a notarized consent letter mentioning the upper floor of the slum. At least one agreement should be from before the cut-off date, along with a recent purchase agreement.
- Priority 4: Aadhar card, passport, ration card, driving license, or voter ID card mentioning the address on the upper floor of the slum before the cut-off date.
- Priority 5: An affidavit from the family head living on the upper floor, along with certification from the family head (or eligible family head) of the ground floor slum (as per Government Decision No. Slum Rehab Scheme -1001/Matter No. 125/14/Slum Improvements-1 dated May 16, 2015, or Government Decision No. Slum Redevelopment Scheme -0810/Matter No. 96/2018/Slum Rehab Project-1 dated May 16, 2018) with a signature stating that it is true and accurate
